An initial assessment of current abilities, goals and the home environment, forming the basis of a personalised rehabilitation plan.
Regular, one-to-one occupational therapy sessions with practical goals, reviewed and adjusted as progress is made.
Who this service may help
Bespoke rehabilitation may be appropriate following illness, injury, a hospital stay or a change in health—particularly when someone needs more than a one-off assessment.
Loss of confidence after a fall or hospital admission
Everyday activities becoming harder
Reduced mobility or difficulty with safe transfers
A need to rebuild routines at home
Regular, goal-focused occupational therapy
Family or carers needing a consistent plan
A plan built around the person
The frequency and duration of sessions are agreed individually. Recommendations are practical, clearly explained and adjusted as progress is made.
The aim is safer participation, greater confidence and as much independence as possible.
What happens
Before
The goals and current difficulties are discussed so the support is shaped around the person.

How many sessions are included?
This is agreed individually based on your needs and reviewed as progress is made.
